Schedule: Monday and Wednesday - 5PM to 10PM

Advanced Care Partners is seeking a Registered Nurse (RN) to provide individualized care for medically fragile children and adults in home and school settings. This position offers a more personalized care model, allowing for stronger patient outcomes and professional satisfaction.

Responsibilities

Provide skilled nursing care (feeding tubes, tracheostomies, ventilators, central lines)
Monitor patient progress and respond to changes in condition
Educate patients and families on treatment plans
Maintain a safe and clean care environment

Qualifications

Active RN license in Georgia or Nurse Licensure Compact
A minimum of one year of RN experience
Current Basic Life Support (BLS) certification through the American Heart Association (AHA) or equivalent
Valid PPD skin test

Physical Requirements: Ability to stand or sit for extended periods (up to 12 hours); ability to bend, lift, and assist with patient mobility; lift up to 50 pounds or more; fine motor skills for clinical tasks
