Job summary

MEP Health in Cleveland, OH seeks a compassionate Registered Nurse (RN) to provide patient-centered home health care, including in-home visits and telehealth consultations. Active RN licensure and CPR are required.

You will conduct assessments, review medications and treatment plans, perform interventions (e.g., wound care, meds administration, point-of-care testing), and document encounters accurately. Travel locally up to 100% and lift up to 50 lbs; flexibility to work in diverse settings.

Qualifications & Skills: Active RN licensure, 3+ years of direct patient care experience, CPR certification, and proficiency with electronic health records and telehealth platforms. Experience in home care, hospice, palliative, or geriatric care preferred. Strong communication, organizational, and decision-making skills, with ability to work independently and travel locally.

Additional Details: No supervisory duties; travel up to 100% locally; ability to lift up to 20-50 lbs; flexible to work in diverse settings.
