Job summary

The VIOS Clinic is seeking a compassionate Registered Nurse (RN) for a home health role providing patient-centered care in patients’ homes, across Ohio. You will perform in-home health assessments, wound care, medications, and point-of-care testing.

You will coordinate with physicians and care teams, document thoroughly, and support telehealth and education while traveling locally to patient homes.

Qualifications

• Active RN licensure in the state; compact license preferred.
• Minimum 3 years in direct patient care, ideally home care, hospice, or geriatrics.
• CPR certification and valid driver’s license with auto insurance.

Responsibilities

• In-home assessments, health evaluations, and safety checks.
• Clinical interventions including medication administration, wound care, and point-of-care testing.
• Documentation, care transitions, and patient education.
• Collaborate with physicians, nurse practitioners, and interdisciplinary teams.
• Support telehealth, attend team meetings, and use electronic health records.
