• Responsibilities: Collaborate with multidisciplinary teams to evaluate patient status, develop and implement care plans, manage health conditions, and coordinate referrals to community resources. Deliver holistic care addressing medical, emotional, and spiritual needs, ensuring dignity and comfort during final stages of life. Monitor and communicate patient conditions, participate in interdisciplinary planning, and support families through challenging times.
• Qualifications & Skills: Valid RN license in Maine or from a compact state, with at least one year of nursing experience preferred; new graduates encouraged to apply. Strong critical thinking, decision-making, and minimal supervision skills required. Proficiency with electronic medical records and a valid driver’s license are necessary.
