Job summary

Position:

Registered Nurse (RN) Home Health

Responsibilities:

Provide patient-centered care in the home setting, including assessment, treatment, education, and management of health conditions. Collaborate with multidisciplinary teams to evaluate patient status, develop and implement care plans, and coordinate referrals to community resources. Promote continuity of care, prevent illness, and support patient health outcomes. Manage a manageable caseload with support from triage and education teams, ensuring quality care with limited on-call and after-hours duties.

Qualifications & Skills:

Active Maine RN license or ability to obtain, at least 1 year of acute or med/surg nursing experience, preferred ED, ICU, CCU, or home care background. Valid driver’s license with a clean driving record, strong technology skills, and experience with electronic medical records (EMR) preferred.
