Responsibilities

  • Monitor patient care plans, ensuring compliance with medical regulations and home health standards.
  • Coordinate patient admissions, discharges, and transfers with complete and up-to-date documentation.
  • Collaborate with field nurses, aides, and staff to assign cases and manage schedules.
  • Provide education to patients and families regarding home health services, treatment plans, and medication management.
  • Assist in developing and implementing quality control procedures to ensure high-quality patient care and regulatory compliance.
  • Collaborate with billing to review patient records, ensure proper coding, and resolve billing or insurance issues.
  • Maintain accurate patient files, clinical documentation, and compliance reports.
  • Help develop and update company policies reflecting best practices in home health care.
  • Serve as primary contact for patient and family concerns and resolve inquiries.
  • Assist in training new clinical staff and provide ongoing education on documentation standards and regulatory changes.

Responsibilities

What You\'ll Do:

  • Monitor patient care plans, ensuring compliance with medical regulations and home health standards.

Qualifications

What You\'ll Bring:

  • Ability to work in a professional, ethical, and respectful manner.
  • Ability to provide personal care for non-ambulatory clients, use a Hoyer lift, maneuver wheelchairs, and transport clients to community events and social activities.
  • Strong communication skills, oral and written; strong interpersonal skills.
  • Ability to anticipate and resolve problems demonstrating good judgment.

Education and Experience:

  • Current Georgia RN license.
  • Current CPR/First Aid certification.
  • One year of clinical nursing experience.
