Job Details

We are hiring a Registered Nurse (RN) – Home Health to provide 1:1 patient care in a field-based setting.

This role is ideal for a Home Health RN, Med-Surg RN, ICU RN, ED RN, or RN Case Manager seeking more autonomy, flexibility, and meaningful patient relationships.

Key Responsibilities

• Deliver skilled nursing care in patients’ homes
• Perform patient assessments and develop care plans
• Provide wound care, infusions, and catheter/line management
• Complete OASIS documentation and maintain accurate EMR records
• Educate patients and families on treatment plans
• Coordinate care with interdisciplinary teams
• Manage schedule and travel within assigned territory

Requirements

• Active RN license (North Carolina)
• 1+ year RN experience (Med-Surg, ICU, ED, or Home Health)
• Strong clinical skills (wound care, IVs, patient assessment)
• Ability to work independently in a field setting
• Reliable transportation and auto insurance

Preferred

• Home Health or OASIS experience
• Case management background
