Registered Nurse (RN) - Home Health Case Manager - Southwest Suburbs

Join Advocate Health as a Registered Nurse (RN) - Home Health Case Manager in the Southwest Suburbs, including Oak Lawn, Burbank, Justice, Bridgeview, Hickory Hills, Willow Springs, and Chicago Ridge.

Position Details

Hours: Days, with weekend and holiday rotation. This position offers a sign-on bonus of up to $10,000, subject to taxes, for new hires employed at the time of payout. Must be able to work one summer and one winter holiday and rotate on call schedule.

Job Summary

The Advocate Home Health division provides Illinois' largest home health and hospice services. The RN will deliver home care to the homebound, establishing strong relationships with patients and caregivers to maximize their potential. The role involves coordinating medical needs with the team, including social workers, CNAs, and physicians, to help patients remain at home and avoid rehospitalization.

Responsibilities

• Document patient care and communicate with the care team via point-of-care documentation.
• Visit private residences, including those inaccessible to individuals with disabilities.

Qualifications

• Graduate from an accredited nursing school
• At least one year of medical-surgical experience in an acute or subacute setting
• Current Illinois RN license and CPR certification
• Maintain 10 continuing education credits annually
• Ability to lift up to 35 pounds without assistance and assist with ambulation of patients up to 200 pounds

Physical & Work Environment

• Ability to work weekends, holidays, and various shifts, including on-call duties
