Registered Nurse (RN) - Home Health Case Manager (Red Team)

This role services patients in zip codes 32960, 32967, 32963 (south), 34949, working in Indian River County. Responsibilities include patient assessments, care planning, coordination with interdisciplinary teams, and supervising LPNs and HHAs. Must work at least one weekend per month and one holiday per season.

Minimum Requirements:

• Current RN license in Florida
• At least one year of home health experience
• Knowledge of Home Care Home Base and Oasis documentation
• Case management experience is a plus
