This position will service patients in the following zip codes: 32960, 32967, 32963 (south), 34949.

The VNA is seeking a full-time RN to work in our Indian River County Home Health division. Responsibilities include overseeing patient caseloads, conducting initial patient evaluations, creating care plans, and coordinating services with the interdisciplinary team. The role involves performing initial assessments, ongoing patient monitoring, providing direct care, and collaborating with physicians.

Responsibilities
  • Assess client environment including family support to develop and establish care plan for patient, education about care and treatment for patient, utilizing family members or designated persons
  • Conduct initial assessment (admission) and establish plan for treatment to obtain thorough physical, psychosocial and environmental data; establish and follow a care plan which identifies problems (nursing diagnosis) and evaluate outcome within a certain time frame
  • Conduct ongoing assessment of patient response to treatment, medication, and teaching with intervention as needed. Revise care plan as needed
  • Provide direct patient care and facilitate focused communication with client and family, establishment of the nursing care plan, interventions, and evaluate outcome
  • Coordinate all services and collaborate with the interdisciplinary team as needed
  • Communicate with physicians to confirm, update patient’s plan of treatment, and provide regular status reports
  • Maintain and update clinical records according to agency policy, State and Federal guidelines, completing documentation daily and timely
  • Schedule clients weekly, updating as needed
  • Supervise and instruct LPNs and HHAs in accordance with regulations
  • Work a minimum of one weekend per month, and one summer and one winter holiday
Qualifications
  • Current RN License in Florida
  • At least one year of home health experience
  • Knowledge of home care software (Home Base, PointCare)
  • Knowledge of Oasis documentation
  • Case management experience is a plus
