Advocate Health is seeking an RN Case Manager for Home Health to provide skilled nursing care in patients' homes across the Waukegan, Zion, and Winthrop Harbor area. You will manage caseloads, coordinate care plans, and collaborate with an interdisciplinary team to deliver patient-centered care.

This full-time position offers a 8:00 AM - 4:30 PM schedule, Monday–Friday, with weekend and on-call rotations. A $3,000 sign-on bonus is available for external candidates, with internal staff not

Competenze

• Current Illinois Registered Nurse (RN) license.
• Graduate of an accredited school of nursing.
• Minimum one year of medical-surgical nursing experience.
• Current CPR certification.
• Valid driver's license and reliable transportation.
• Strong communication, critical thinking, and organizational skills.
• Basic computer and documentation proficiency.
• Previous home health or case management experience (preferred).
• Bachelor's degree in Nursing (BSN) (preferred).

Registered Nurse (RN) Case Manager - Home Health

Service Area: Waukegan, Zion, and Winthrop Harbor, IL

Join our Home Health team as an RN Case Manager, providing skilled nursing care to patients in the comfort of their homes. In this role, you will manage a patient caseload, coordinate care plans, and collaborate closely with LPNs and an interdisciplinary team to deliver high-quality, patient-centered care.

Schedule

• Full-time, Monday-Friday
• 8:00 AM - 4:30 PM
• Weekend rotation required
• On-call rotation required

$3,000 Sign-On Bonus Available for External Candidates!
Internal Advocate Health team members are not eligible for the sign-on bonus. Additional eligibility requirements may apply.

Position Overview

The RN Case Manager is responsible for assessing, planning, coordinating, implementing, and evaluating care for patients receiving home health services. This role serves as the primary clinical coordinator for assigned patients and works closely with physicians, LPNs, therapists, social workers, and other healthcare professionals to ensure optimal outcomes.
