Schedule: Hourly, Full-Time

Shift: Friday, Saturday, Sunday, Monday (4, 10 hour shifts) or Saturday, Sunday plus 3 variable Weekdays (5, 8 hour shifts)

Location: Kansas City Area - Service area can include Leavenworth, Johnson, Wyandotte, Franklin, Douglas and Miami Counties in Kansas.

The Role You’ll Contribute: The Home Health Registered Nurse (RN) Care Manager is responsible for coordinating and directing home care based on patient needs, managing a caseload, developing care plans, educating patients and families, and monitoring outcomes. The RN ensures proper referrals, interdisciplinary communication, and documentation.

• Assess, plan, implement, and evaluate patient care for a caseload of home health patients.
• Prioritize and adapt care to changing patient needs, optimizing visit schedules.
• Formulate individualized care plans with patients, families, and physicians, setting realistic goals.
• Communicate changes in patient condition to the healthcare team and facilitate interdisciplinary conferences.
• Maintain accurate and timely clinical documentation.

The Expertise And Experiences You’ll Need To Succeed:

• Current RN license in the state of practice.
• Valid driver’s license and insurance.
• Active American Heart Association BLS certification.
• Minimum 1 year relevant clinical RN experience.
• Preferred: Bachelor’s degree in nursing, recent home health experience, Home Health Case Manager Certification, COS-C certification.
