Job summary

Buena Vista Home Health is seeking a Registered Nurse to join our Home Health team in Thousand Oaks, CA. The RN will plan, organize, and direct home health services, collaborate with an interdisciplinary team to deliver patient-centered care, and build meaningful relationships with patients and caregivers.

Qualifications

• Active California RN license required.
• Minimum 2 years nursing experience, preferably in home health or public health.
• Current CPR certification.
• Valid driver's license and insured, reliable vehicle.
• Ability to lift, transfer, and position patients.

Responsibilities

• Develop, implement, and revise individualized plans of care with patients, caregivers, and providers.
• Provide skilled nursing care, including preventive and rehabilitative services per physician orders.
• Deliver patient and caregiver education to promote independence and safety.
• Maintain accurate and timely clinical notes and documentation.
• Communicate regularly with physicians and healthcare team members regarding patient status.
• Identify and coordinate discharge planning needs.
• Ensure care is delivered in compliance with all regulatory and company standards.
