Job summary

GrandCare Health Services is seeking a part-time Registered Nurse (RN) to provide in-home health services across the San Fernando Valley. You will coordinate and deliver high-quality home health care, developing individualized plans of care and educating patients and families in their own homes.

The role emphasizes flexible scheduling, autonomy with strong clinical support, and opportunities for specialization within orthopedic, neuro, and pain management care.

Qualifications

• California RN license required.
• 2+ years nursing experience; home health preferred.
• CPR certification required.

Responsibilities

• Develop and initiate the plan of care in partnership with patient and caregivers.
• Perform and supervise nursing procedures as ordered in the plan of care.
• Educate patient and caregiver on care needs and safety.
• Identify discharge planning needs and implement prior to discharge.
• Arrange equipment and services as needed for continued care.
• Supervise LPN/LVN and HHAs to ensure quality home health delivery.
