Job summary

Buena Vista Home Health is seeking a licensed Physical Therapist (PT) to provide in-home therapy across the South Coastal Territory, helping patients regain mobility and independence. You will evaluate functional status, develop care plans, educate patients and caregivers, and collaborate with physicians while documenting progress and participating in discharge planning.

This part-time, per-diem role requires CA licensure, CPR certification, reliable vehicle, and at least two years of PT

Qualifications

• Active California Physical Therapy License.
• Minimum 2 years of physical therapy experience (home health preferred).
• Strong communication and organizational skills.
• Current CPR certification; valid driver’s license and insured vehicle.

Responsibilities

• Evaluate patients’ functional status and develop care plans.
• Assess home environments and recommend modifications.
• Provide direct physical therapy treatments.
• Educate patients and caregivers on therapeutic techniques and equipment.
• Monitor progress and adjust treatment plans.
• Collaborate with physicians and interdisciplinary teams.
• Document clinical notes and progress reports.
• Participate in discharge planning.
• Supervise Physical Therapy Assistants per policy and regulations.
