Comfort Home Care in Somerville, MA is seeking a full-time Registered Nurse (RN) for an afternoon/evening weekday schedule. The role offers competitive pay and an uncapped incentive plan, with flexibility, autonomy, and strong support to thrive in both career and personal life.

Qualifications

• Active Registered Nurse (RN) license in Massachusetts.
• One year of nursing experience (home health or psych preferred).
• Reliable transportation for patient visits.

Responsibilities

• Assess and Adapt: Each visit starts with evaluating your patient's current condition, medical needs, and overall well-being.
• Create and Modify Care Plans: Actively shaping your patients' treatment plans based on what they need, adjusting care as conditions change.
• Teach and Support: Provide medication guidance and patient education to help them stay independent at home.
• Communicate and Coordinate: Keep providers, families, and care teams informed to ensure right care at the right time.
• Document and Reflect: Submit notes from home to ensure accurate records for continuity of care.
