Job summary

Transition Home Healthcare is seeking a dedicated Registered Nurse (RN) for home health services in the Houston, TX area. PRN shifts available across Northeast, Southwest, Southeast, and Inner Loop service areas.

The RN will oversee nursing care teams, collaborate with physicians and therapists, plan and direct home care, and educate patients and families on health management and medications. Strong communication and independent work skills are essential.

Qualifications

• Valid RN license in Texas or compact (DS) license.
• Home health experience of at least 1 year.
• Knowledge of home health regulations and compliance.
• Excellent communication and teamwork skills.

Responsibilities
