Job summary

Transition Home Healthcare in Houston, TX is seeking a dedicated Registered Nurse (RN) for PRN home health assignments in the Inner Loop area. You will oversee patient care, collaborate with physicians and therapists, and direct home care services to ensure quality outcomes.

Responsibilities include conducting patient assessments, developing care plans, educating families, and maintaining accurate clinical notes.

Qualifications

• Valid RN license in Texas or compact license.
• Home health experience of at least 1 year.
• Knowledge of home health regulations and compliance.
• Experience with HCHB EMR is preferred.
• Excellent communication and people skills are essential.
• Ability to work independently and as part of a team.

Shifts Available: PRN

Service Areas with Openings: Inner Loop

Responsibilities

• Oversee nursing care team.
• Communicate and collaborate with physicians, therapists, and other healthcare professionals to ensure cohesive and comprehensive patient care.
• Plan, organize, and direct home care services.
• Conduct assessments of patient care needs and develop care plans.
• Educate patients and their families on managing their health conditions, medication regimens, and any necessary lifestyle changes.
• Prepare timely, complete, and accurate clinical notes.

Qualifications

• Valid RN license in the State of Texas or compact license.
• Home health experience (at least 1 year).
• Knowledge of home health regulations and general healthcare compliance.
• Experience with Home Care Home Base (HCHB) EMR (preferred).
• Excellent communication and people skills are essential.
• Ability to work independently and as part of a team.
