Job summary

Transition Home Healthcare in Houston, TX is seeking a qualified Registered Nurse for home health with PRN shifts to support patient care across service areas.

You will oversee nursing care, collaborate with physicians and therapists, assess patient needs, plan and direct home care services, and educate families.

Texas RN license or compact, minimum 1 year home health experience, strong communication, and ability to work independently and as part of a team.

Qualifications

• Valid RN license in the State of Texas or compact license.
• Home health experience (at least 1 year).
• Knowledge of home health regulations and general healthcare compliance.
• Experience with Home Care Home Base (HCHB) EMR (preferred but not required).
• Excellent communication and people skills are essential.
• Ability to work independently and as part of a team.

Responsibilities

• Oversee nursing care team.
• Communicate and collaborate with physicians, therapists, and other healthcare professionals to ensure cohesive and comprehensive patient care.
• Plan, organize, and direct home care services.
• Conduct assessments of patient care needs and develop care plans.
• Educate patients and their families on managing their health conditions, medication regimens, and any necessary lifestyle changes.
• Prepare timely, complete, and accurate clinical notes.
