Job summary

UChicago Medicine AdventHealth is seeking a Registered Nurse to provide comprehensive patient care in Hinsdale, IL. The role includes coordinating multidisciplinary care, administering medications, and supporting discharge planning.

The position requires an Associate's Degree in Nursing (Bachelor's preferred) with RN licensure and BLS. Schedule is full‑time day shift with travel between area facilities as needed.

• Monitors patient condition and communicates changes promptly.

• Administers medications and solutions as required.
• Performs holistic assessments considering age and status.
• Documents patient status and care phases thoroughly.
• Assists in discharge planning with the healthcare team.
• Maintains safe, clean, and compliant work environment.
• Leads and mentors staff to uphold care quality.
• Develops positive relationships within the practice and departments.

• Monitors, records, and communicates patient condition as appropriate.
• Serves as the primary coordinator of all disciplines for well-coordinated patient care.
• Notes and carries out physician and nursing orders accurately and promptly.
• Conducts holistic, individualized assessments of each patient considering age, physiological, and psychological status.
• Administers medications and solutions as required.
• Documents all phases of patient status during care.
• Participates in the transfer of patient information to ensure continued quality care.
• Leads and mentors staff to ensure optimal performance and competency in care delivery.
• Maintains a safe and clean working environment by complying with policies, procedures, rules, and regulations.
• Develops and maintains positive working relationships within the practice and other departments.
• Asesses and coordinates patient's discharge planning needs with members of the healthcare team.
• Other duties as assigned

Job Description:

Up to a $10,000Sign on Bonus Available

Schedule: Full-Time/Day
Shift: Day Shift Monday-Friday 8am to 5pm with e/o weekend or call rotation.
Location: Barington, Schaumburg, Batavia, Carol Stream, Hoffman Estates, Itasca, Medinah, Roselle, St Charles, and Steamwood Area with travel to LaGrange Hospital for meetings/Orientation etc.

Mileage reimbursement provided: $0.76 per mile

Flexible scheduling with Full Time, Part Time and PRN available

Company provided phone and laptop/iPad

• 1+ acute care clinical experience [Preferred]
• 1+ home health experience [Preferred]

• Basic Life Support - CPR Cert (BLS) [Required]
• Driver's License (DL) [Required]
• Auto Insurance (AL) [Required]
