Job summary

Mental Health Association of Rochester is seeking a compassionate Registered Nurse (RN) for a home health role involving in-home visits, telehealth consultations, and administrative tasks.

The role requires an active RN license, at least 3 years of direct patient care, CPR certification, and the ability to travel locally. You will conduct safety assessments, perform wound care and med administration, support transitions, and educate patients and caregivers in a collaborative team setting.

Qualifications

• Active state RN licensure; compact license preferred.
• At least 3 years direct patient care experience; telehealth and home care experience preferred.
• CPR certification and valid driver’s license with auto insurance.
• Proficiency with EHR, Microsoft Office, and telehealth platforms.
• Strong communication, organizational, and decision-making skills.
• Ability to work independently, handle administrative tasks, and travel locally.
• Experience with diverse and underserved populations is a plus.

Responsibilities

• Conducting comprehensive home safety evaluations and physical assessments.
• Performing clinical interventions such as medication administration, wound care, point-of-care testing, and other nursing procedures.
• Supporting care transitions, end-of-life discussions, and addressing HEDIS quality gaps.
• Documenting patient encounters accurately and timely.
• Collaborating with interdisciplinary care teams to develop and implement individualized care plans.

• Participating in team meetings and community engagement.
