Job summary

Interim HealthCare Inc. is seeking a Registered Nurse (RN) to deliver skilled, compassionate home health care for patients in South Carolina. You will support recovery and long-term health by coordinating with physicians and a multidisciplinary team, while documenting care and adhering to OASIS requirements.

You will assess patients, develop personalized plans, educate families, and ensure safe discharges, all while ensuring quality outcomes in a trusted home-care setting.

Qualifications

• Graduate of an accredited Registered Nurse (RN) program
• Active South Carolina RN license or Compact License
• Minimum one year of nursing experience
• Experience in Home Health, Medical Surgical, Critical Care, or Case Management preferred
• Familiarity with OASIS documentation preferred

Responsibilities

• Provide skilled, patient centered care in the home while partnering with physicians and the interdisciplinary team to support safe recovery and long-term health
• Perform comprehensive patient assessments, including accurate OASIS documentation
• Develop, implement, and adjust individualized plans of care
• Monitor patient condition and communicate changes promptly to physicians to prevent complications
• Educate patients and caregivers on medications, treatments, disease management, and home safety
• Coordinate care with therapists, aides, and other providers to ensure seamless, compliant care
• Prepare patients and families for safe, successful discharge and document care accurately in the EMR
