Job summary

Elara Caring in the Bronx, NY is seeking a Registered Nurse to deliver high-quality personal home care. You will work independently, assess patients at home, guide caregivers, and ensure care is delivered safely and effectively.

Qualifications

• Active RN license in New York state.
• Minimum 1 year nursing experience; home care preferred.
• Graduate of an accredited nursing program (Certificate/Diploma/AS/BSN).
• Strong clinical assessment and decision-making skills.
• Excellent communication and organizational abilities.

Responsibilities

• Conduct in-home patient assessments and develop care plans.
• Supervise and support caregivers to ensure quality care delivery.
• Perform start-of-care and supervisory visits.
• Monitor patient condition and adjust care plans as needed.
• Educate patients, families, and caregivers on care delivery.
• Ensure compliance with state, federal, and company standards.
• Maintain accurate and timely clinical documentation.
• Partner with clinical teams and case managers to coordinate care.
