Job summary

Elara Caring is seeking an experienced Registered Nurse (RN) to deliver patient care across the home health continuum. You will provide high-quality, compassionate nursing services in patients' homes and report to the Clinical Supervisor.

Requirements include an accredited nursing program degree and an active RN license, with at least 1 year of clinical care experience. Home health or hospice background is preferred, and travel up to 50% is required.

Qualifications

• Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing.
• Current State License as a Registered Nurse (RN).
• 1 year of clinical care experience.
• Experience in hospice or home health preferred.
• Ability to frequently lift, push, pull, and support up to 50 pounds with patient positioning and equipment movement.

Responsibilities

• Provide patient care across the home health continuum.
• Document care in the patient record and communicate with the Clinical Supervisor.
• Coordinate with families and other care team members to ensure seamless service.

What is Required?
