The Home Care Admissions RN is the initial point of contact for patients entering home care services. Assesses the patients' eligibility for home care, explains services and benefits, completes admission paperwork, and ensures a smooth transition to home care services. Works closely with interdisciplinary teams to provide comprehensive and compassionate care.
