Pay range for this position is $36.23 - $53.02/hour. Actual pay is based on years of licensure. This is an Hourly position.

**Eligible for a $15,000 Sign-On Bonus with a two-year work commitment to OSF Home Care - external candidates**
OSF will invest in you from day 1! Candidates with 5+ years of direct experience will receive 24 hours of Vacation/Holiday time frontloaded on day 1, in addition to our generous Paid Time Off plans.
The Registered Nurse Home Care is responsible for a scope of care that includes all ages from infancy through geriatric. The RN demonstrates the knowledge and skills necessary to provide appropriate patient care for the ages served. Coordinates interdisciplinary care, which may include nursing, therapy, social work, home care aides, home medical equipment, and home infusion services as needed. The RN Case Manager makes decisions regarding the home health care during the patient's episode of care, providing patient-centered care and utilizing critical thinking to ensure positive outcomes. Responsibilities include care coordination, visit pattern utilization, outcome monitoring, resource management, and episode management.
