Communitas, Inc. in Wakefield, MA is seeking a Residential RN Healthcare Coordinator to oversee the healthcare and wellbeing of individuals in our residential program and community settings.
The role requires a Massachusetts RN license and 3–5 years of experience in community health, ideally with adults with developmental disabilities. MAP training is valued; background checks, valid license, and transportation are required. This is a full-time role (~30 hours/week).

Position: Residential RN Healthcare Coordinator
Location: Wakefield, MA
Pay rate: $68,640 annual salary
Hours: Full-time, 30 hours per week
Schedule: This position will typically operate on a Monday – Friday schedule; may be required to work evenings and weekends in emergency healthcare needs; must have flexibility as schedule may vary to meet needs of agency and the Individuals we support
The Health Care Coordinator will be responsible for the oversight of the overall healthcare and well-being of individuals who reside in our Residential Program group homes and independent apartment settings. We are looking for someone with a strong background and experience in working with adults with Developmental and Intellectual Disabilities.
