Job summary

Communitas, Inc. is seeking a Specialized Family Support Coordinator in Wakefield, MA. This role focuses on home visits, assessment of family needs, and connecting families with community supports and DDS resources.

The position offers full-time hours and a competitive hourly rate. Responsibilities include developing Family Support Plans, coordinating services, and advocating for access to SSI, SNAP, MassHealth, housing, and transportation.

Qualifications

  • Bachelor's degree in a human services–related field; Master's degree preferred.
  • Minimum of two years of direct family support experience.
  • Valid Driver’s License and acceptable driving record.
  • Must be able to pass company background and reference checks.

Responsibilities

  • Conduct home visits to assess family needs and identify community supports and benefits.
  • Develop and implement a Family Support Plan to support growth and independence.
  • Assist families in accessing services and supports in the community.
  • Provide trauma-informed care when addressing mental health or behavioral challenges.
  • Help families obtain benefits such as SSI, SNAP, MassHealth, housing and transportation.
  • Coordinate with clinical providers to support coordinated care.
  • Assist with DDS flexible funds for short-term needs and connect with schools and hospitals.

Summary & Responsibilities


  • Conducts home visits to assess family needs and identifies ways to access community supports, benefits, and DDS supports based on those needs.

  • Facilitates the development of a Family Support Plan that incorporates the individual’s needs and supports their growth and independence.

  • Supports families in accessing services and supports available within the community.

  • Practices good judgement in responding to families navigating mental health or behavioral challenges by providing trauma informed care.

  • Assists families in obtaining state and federal entitlement benefits, including SSI, SNAP, MassHealth, subsidized housing, and transportation benefits.

  • Assists families in accessing clinical and medical supports, including behavioral supports, therapy, counseling, nursing, and related services.

  • Collaborates with clinical and medical providers to support coordinated care for individuals and families.

  • Facilitates the use of DDS flexible funds to assist families with short‑term needs that cannot be met through existing services and supports.

  • Works with local hospitals, schools, and community service organizations to identify potential support personnel, including respite providers, PCAs, and skills instructors.



Qualifications


  • Bachelor's degree in a human services–related field; Master's degree preferred (including but not limited to social work, education, psychology, public administration, or public health)

  • A minimum of two years of experience providing direct support to families.

  • A passion and dedication to supporting our people is a must

  • Valid Driver’s License and acceptable driving record

  • Must pass company background and reference checks
