Job summary

Communitas, Inc. in Wakefield, MA is seeking a Registered Nurse Health Care Coordinator. The role oversees healthcare and well-being for individuals in residential group homes and independent settings.

The position requires a MA RN license and 3–5 years’ community health experience with adults with developmental disabilities. The role includes staff training on protocols, medical consultations, MAP trainings, audits, and visits to homes and hospitals.

Qualifications

  • Must have a current Massachusetts license as a Registered Nurse (RN).
  • 3–5 years’ experience in a community health setting; preferably with adults with developmental and intellectual disabilities.
  • Certified MAP Trainer is a plus but training is also available.
  • Successful candidates will pass a background check including references, CORI, and fingerprinting.
  • A passion and dedication to supporting our people is a must.

The role!

Position: Residential RN Healthcare Coordinator

Location: Wakefield, MA

Pay rate: $68,640 annual salary

Hours: Full-time, 30 hours per week Schedule: This position will typically operate on a Monday - Friday schedule; may be required to work evenings and weekends in emergency healthcare needs; must have flexibility as schedule may vary to meet needs of agency and the Individuals we support

Summary & Responsibilities

The Health Care Coordinator will be responsible for the oversight of the overall healthcare and well-being of individuals who reside in our Residential Program group homes and independent apartment settings. We are looking for someone with a strong background and experience in working with adults with Developmental and Intellectual Disabilities.

  • Must have at least 3 to 5 years’ experience in a community health setting; preferably with adults with developmental and intellectual disabilities.

  • A passion and dedication to supporting our people is a must
  • Valid Driver’s license, reliable transportation, and acceptable driving record
  • Must pass company background and reference checks
