Job summary

Tender Care Home Health & Hospice seeks a dedicated Registered Nurse (RN) for full-time or PRN positions in Las Cruces, NM. You will plan, organize, and direct home care services, coordinating with physicians, social workers, and therapists to tailor patient-centered care in home settings.

You will educate families, monitor patient status, and ensure regulatory compliance while supporting comfort and independence for patients in their own homes.

Qualifications

• Valid RN license in NM.
• Experience in home health or hospice preferred.
• Ability to work independently in patients' homes.

Responsibilities

• Conduct comprehensive assessments of patients in their homes and manage pain, medications, and family supports.
• Develop individualized care plans with the interdisciplinary team.
• Provide skilled nursing care including wound, catheter, infusion, and palliative care as ordered.
• Educate patients and families on disease management and symptom control.
• Monitor patient progress and adjust plans with the team and PCPs.
• Coordinate care transitions between settings to ensure continuity.
• Offer emotional support and bereavement counseling to patients and families.

Job Description Summary:

The Registered Nurse (RN) is responsible for planning,organizing, and directing home care services while directly reporting to theClinical Supervisor(s). They possess extensive nursing experience with a focuson community health education and practice. The RN utilizes community resourcesto develop and implement services tailored to meet the needs of individuals andfamilies within their homes and communities.

Job Duties (include, but not limited to):

• Conductcomprehensive assessments of patients' physical, emotional, andpsychosocial needs in their homes, including pain assessment andmanagement, medication reconciliation, and evaluation of their supportsystems.
• Developindividualized care plans in collaboration with the interdisciplinaryteam, including physicians, social workers, and therapists, to addresspatients' goals, preferences, and needs.
• Provideskilled nursing care and interventions based on assessment findings andphysician orders, including wound care, catheter care, infusion therapy,and palliative care measures.
• Educatepatients and their families on disease management, medicationadministration, symptom management, and end-of-life care to promoteunderstanding and autonomy in decision-making.
• Monitorpatients' condition and response to treatment, adjusting care plans asneeded and communicating changes to the interdisciplinary team and primarycare providers.
• Coordinatecare transitions between healthcare settings, ensuring continuity of careand adherence to the plan of care during hospitalizations, rehabilitationstays, and hospice admissions.
• Provideemotional support and counseling to patients and families facing seriousillness, end-of-life care, and bereavement, fostering coping mechanismsand facilitating difficult conversations.
• Collaboratewith community resources and agencies to address patients' socialdeterminants of health, such as transportation, housing, and financialassistance, to optimize their well-being.
• Documentpatient assessments, interventions, and outcomes accurately and timely inelectronic medical records, adhering to regulatory and agency standardsfor documentation and reimbursement.
• Participatein interdisciplinary team meetings, case conferences, and qualityimprovement initiatives to evaluate patient outcomes, identifyopportunities for improvement, and enhance care delivery.
• Adhereto ethical and legal standards of nursing practice, including maintainingpatient confidentiality, respecting patients' rights, and advocating fortheir best interests.
• Stayupdated on evidence-based practices, clinical guidelines, and regulationsrelated to home health and hospice care through continuing education,professional development, and participation in relevant seminars orconferences.
• Serveas a resource and mentor for less experienced staff, providing guidance,support, and clinical expertise to promote professional growth andexcellence in patient care.
• Ensurecompliance with agency policies, procedures, and regulatory requirements,including infection control protocols, HIPAA regulations, and Medicareconditions of participation.
• Fosterpositive relationships with patients, families, caregivers, and healthcarepartners through effective communication, empathy, and culturalsensitivity, promoting a collaborative approach to care delivery.
