Job summary

Tender Care Home Health & Hospice is seeking a compassionate Physical Therapist to join our Las Cruces home-health team. This full-time role provides personalized therapy in patients' homes, aiming to improve safety, comfort, and functional outcomes.

The successful candidate will be a licensed Physical Therapist in NM with strong communication and documentation skills, able to travel to patient homes and support interdisciplinary care. PRN opportunities available.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Licensed Physical Therapist in the state of practice.
• Minimum of 2 years’ experience preferred (PT/PTA).
• Current CPR certification.
• Strong communication, documentation, and organizational skills.
• Reliable transportation and ability to travel to patient homes.

Responsibilities

• Evaluate patients in home health settings and assist physicians in developing care plans.
• Provide skilled physical therapy services including ROM testing and exercise programs.
• Educate patients, caregivers, and team members on therapy protocols.
• Conduct re-evaluations and adjust plans to meet evolving patient needs.
• Maintain timely and accurate clinical documentation.
• Supervise PTAs and Home Health Aides as required.
• Participate in interdisciplinary team meetings and ongoing education.
• Ensure compliance with agency, HIPAA, and infection control policies.

We’re seeking a compassionate and skilled Physical Therapist to join our home health team. This role is responsible for providing personalized physical therapy services to patients in their homes, ensuring safety, comfort, and improved function.

Key Responsibilities
