Horizon House, Inc. in Claymont, Delaware is seeking a qualified Registered Nurse (RN) to join our ACT Case Management Hope team.
The role involves coordinating behavioral and physical health care needs, developing recovery plans with participants and staff, and managing a caseload with timely documentation. This position requires Delaware RN licensure (or participating in the Nursing Licensure Compact including DE) and strong communication; mental health experience is preferred.

We are seeking qualified Registered Nurse (RN) interested in joining our ACT Case Management Hope team at our Claymont, Delaware location.
Work Schedule: Monday – Friday, 8:30-4:30 PM
To support the onboarding and training experience, this position includes in-person orientation during the first four (4) workdays (Monday through Thursday) at our Philadelphia location. Mileage reimbursement is available.
Board Certified Licensed RN in the state of Delaware; or participating in the Nursing Licensure Compact to include the state of DE.
