Horizon House, Inc. is seeking a Residential Clinician to provide coordination and guidance to participants transitioning from residential services to more independent environments. Responsibilities include case management, conducting participant assessments, and developing recovery plans.

The position is scheduled for 40 hours per week, consisting of two 8-hour shifts. Work schedules are developed based on program and operational needs.
The Residential Clinician will provide coordination and guidance, counseling, and encouragement to participants as they transition from residential services to a more independent environment. The Residential Clinician will also implement and document participant’s progress in accordance with individualized rehabilitation plans and ensure a safe, healthy and secure environment which promotes growth and development.
