Home Care Providers is seeking an experienced Registered Nurse (RN) to join Corporate Clinical Operations in Indianapolis. The role blends hands-on home health care with clinical operations, QA, and multi-state support.

Expect travel across service areas, mentorship of clinicians, and collaboration with leadership to improve patient outcomes. The position requires an active Indiana RN license, 3+ years in home health, OASIS proficiency, and strong regulatory knowledge.

Qualifications

• Active Indiana RN license.
• 3+ years of Home Health nursing experience.
• Strong knowledge of Medicare Conditions of Participation.
• OASIS proficiency.
• Experience with regulatory compliance and quality assurance.

Responsibilities

• Perform skilled nursing visits in patients' homes and complete SOC, ROC, Recertification, Follow-Up, and Discharge assessments.
• Complete accurate OASIS documentation and develop Plans of Care.
• Coordinate care with physicians and interdisciplinary teams; educate patients and caregivers.
• Ensure regulatory compliance and timely charting.
• Support multiple branches with clinical operations and onboarding of new clinicians.
• Participate in process improvements and standardize clinical practices across locations.
