Job Responsibilities of a Registered Nurse (RN) Clinical Program Post-Cardiac Transplant Coordinator:
Patient Care Coordination: Manages and coordinates care for post-transplant cardiac patients across the continuum—from post-transplant education through discharge and long-term follow-up.
Education & Training: Provides comprehensive education and support to cardiac transplant patients, families, and caregivers, ensuring they understand medication management, lifestyle changes, and follow up needs.
Clinical Monitoring & Management: Monitors patients for complications and overall clinical status, collaborating closely with the multidisciplinary team to optimize outcomes.
Regulatory Compliance & Documentation: Ensures adherence to institutional protocols and regulatory standards while maintaining accurate and timely documentation in patient records and databases.
