Alternate Solutions Health Network, LLC is seeking a Registered Nurse (RN) Clinical Field Staff Liaison in the Kettering area. The role supports field clinicians, monitors quality of home health services, and ensures compliance with clinical processes.

The ideal candidate has 3 years of acute care RN experience and 1+ year home health exposure, with compact licensure and Ohio license. Hybrid schedule after 90 days, with some remote work and office presence required.

Qualifications

• Registered Nurse with current license in the state of employment.
• Compact licensure, for states that participate, must be obtained within first 30 days of employment for additional support in all ASHN affiliated agencies.
• Three years of experience as an RN in an acute care setting.
• Minimum one-year home health experience preferred.
• Ability to communicate effectively with patients, families, physicians and coworkers.
• Patience, calm demeanor, and compassionate patient care.
• Knowledge of nursing best practices and ability to follow directions as a team member.
• Proficiency with MS Office applications (Word/Excel/PowerPoint/Outlook).

Responsibilities

• Serve as a resource for field staff, referral sources, physicians, patients and their families on clinical issues.
• Facilitate calls to and from Field staff and report concerns to the manager.
• Work in the field with staff and patients as needed.
• Maintain positive relationships with referral sources, physicians and facilities.
• Assist with monthly Case Manager, Nurse and Therapy meetings.
• Ensure Start of Care compliance and quality of home health services.
• Monitor Home Health Aide supervisory visits and focus on outcome monitoring.
• Participate in quality improvement programs in the field.

Registered Nurse (RN) Clinical Field Staff Liaison Schedule: Monday-Friday 8:15AM-5PM with Holiday requirement per rotation Agency: Kettering Home Care - Registered Nurse with current license in the state of employment.

• THE IDEAL CANDIDATE WILL HAVE 1+ YEARS WORKING IN HOME HEALTH. THIS POSITION WILL TRAIN IN OFFICE FOR 90 DAYS AND HAS THE POSSIBILITY TO THEN WORK HYBRID...3 DAYS REMOTE AND 2 IN OFFICE.

SUMMARY

The RN Clinical Field Staff Liaison (CFSL) provides support and acts as a resource to clinicians in the field. The CFSL assists in monitoring compliance quality and safe delivery of services by following a clinical process set by clinical management. The CFSL is responsible for all practices and duties within the scope of practice as outlined by the state.

QUALIFICATIONS & ATTRIBUTES

• Three years of experience as an RN in an acute care setting
• Minimum one-year home health experience is preferred

MAJOR AREAS OF RESPONSIBILITY

• Serve as a resource for field staff, referral sources, physicians, patients and their families pertaining to clinical issues and/or concerns
• Facilitate calls to and from Field staff; uses resources to resolve issues and reports anything of concern to manager
• Work in the field with staff and patients as needed
• Maintains positive working relationships with referral sources including physicians, hospitals, skilled facilities, assisted livings and families
• Assist with monthly Case Manager, Nurse and Therapy meetings
• Ensure Start of Care compliance
• Ensure quality and safe delivery of home health services
• Monitor Home Health Aide supervisory visits
• Focus on Outcome Monitors as assigned
• Implement quality improvement programs in the field
• Completes field visits as needed
• Participates in managing cost of good working with monitoring supply costs and visits per episode
• Knowledgeable of CFSS workflow
• Review relevant reports
• Reviews clinical documentation in accordance with agency protocol and Medicare/Federal guidelines
• Maintains active RN license in state of employment
• Participates in on-call, evening/weekend shifts and provides patient care, per agency needs
• Participates in in-service program development
• Maintains a professional appearance as a representative of the company
• Understands, implements and practices policies and procedures for agency operation
• Other duties as assigned
