Job summary

OSF HealthCare is seeking a Registered Nurse who will protect, promote and restore health by practicing at the highest level of licensure and coordinating care across the continuum. You will collaborate with providers and a multidisciplinary team to support patients, families, and the care plan.

The role emphasizes mentorship, adherence to best practices, and professional discretion, with travel between sites as part of patient care coordination in a mission-driven health system.

Qualifications

• Current IL or MI RN license required.
• American Heart Association (BLS for Health Care Providers).
• BSN degree preferred.
• 6 months experience as an RN in a healthcare environment.
• Electronic Medical Record and Microsoft applications basic proficiency.
• Ability to regularly and timely travel between various sites.

Responsibilities

• Provide care coordination across the care continuum as part of a multidisciplinary team.
• Assess and support physical symptoms, and provide psychological support to patients and families.
• Mentor assigned team members and promote best practices.

Pay range for this position is $35.79 - $53.64/hour. Actual pay is based on years of licensure. This is an Hourly position.
