Job summary

St. Croix Hospice is seeking a Registered Nurse Case Manager to provide 1:1 holistic support for patients and families in their homes. You will collaborate with an interdisciplinary team to develop care plans and guide end-of-life conversations with empathy and professionalism.

The role requires an RN with acute care experience and hospice/home health preferred. Schedule is 8:00am–5:00pm, full-time with on-call duties; benefits include health/dental/vision/life, disability, 401(k) with company

Qualifications

• Requires RN license with an accredited nursing program (AA/BS/MS).
• 1+ years of acute care experience; hospice/home health preferred.
• Develop individualized care plans with patients and families.
• Educate patients and caregivers on care techniques, medications, and symptom management.
