Phoenix Home Care and Hospice is seeking a dedicated RN Case Manager to provide compassionate nursing care to terminally ill clients in Kansas City, MO and surrounding areas. You will collaborate with an interdisciplinary team to develop and implement patient/family centered plans of care, guiding with goals of care discussions and healthcare decisions to improve quality of life.

Qualifications

• Missouri or Compact RN license required.
• Valid driver’s license and auto liability insurance.
• Experience in hospice or home care preferred.

Responsibilities

• Provide education to patients and families about care plans and healthcare decisions.
• Consult with physicians regarding changes in patient condition and plan of care.
• Coordinate hospice care with the care team to meet patient needs.
• Conduct initial and ongoing comprehensive assessments.

• Full Time: Monday - Friday, Days
• Kansas City, MO and surrounding areas
• Pay Range: $75,000 - $90,000

Phoenix Hospice RN Case Managers provide compassionate nursing care to clients and their families dealing with a terminal diagnosis. Will work with the interdisciplinary team to develop and implement a plan of care that is patient/family centered, with goals of care discussions and guidance with healthcare decisions to give quality of life.

Responsibilities

• Provide education to patients/family members regarding the process, plan of care, and offers guidance with healthcare decisions
• Consult with the attending physician and Hospice Medical Director concerning changes in patients condition requiring order changes and/or changes to plan of care
• Coordinate hospice care by working with other team members as well as with the patients/family members to ensure needs are met
• Conducts initial as well as on-going comprehensive assessments
