Ideal candidates hold a Tennessee RN license with AHA BLS and an accredited associate degree; BSN preferred, plus 1 year of experience.

Qualifications

• Current Tennessee or compact state RN license.
• Associate’s degree from an accredited nursing school; BSN preferred.
• AHA Basic Life Support (BLS) certification within 30 days of hire.
• One-year previous experience preferred.

Responsibilities

• Monitors and manages patient care to promote continuity of care and outcomes.
• Consults with nursing staff and multidisciplinary team on patient status and disposition.
• Monitors patient and family satisfaction and assists payors as needed.
• Leads discharge planning and coordinates patient transfers and referrals.
• Performs chart reviews to address service delivery, outcomes, and reimbursement.
• Documents activities and participates in performance improvement and continuing education.

How you'll contribute

• The RN Case Manager monitors and manages patient care to promote continuity of care, optimal patient outcomes, patient satisfaction, cost efficiency, and compliance.
• Consults regularly with nursing staff and the multidisciplinary team to evaluate patient status and appropriateness of care, including admission, length of stay, transfers, and discharge.
• Monitors patient and family satisfaction; responds to questions and complaints from patients, families, and payors.
• Leads and coordinates discharge planning, including patient transfers to other facilities and referrals to community resources; provides discharge education.
• Performs chart reviews to identify and address issues related to service delivery, patient outcomes and satisfaction, compliance, cost, and reimbursement.
• Communicates effectively with providers, payors, and ancillary services to remove barriers and ensure timely care progression.
• Accurately documents case management activities, plans, and outcomes.
• Participates in performance improvement and required continuing education.
• Supports department operations by working alternate shifts/units as needed; may be assigned on‑call.
• Performs other duties as assigned.

What we're looking for

• Current Tennessee or compact state RN license.
• Associate’s degree from an accredited nursing school; BSN preferred.
• AHA Basic Life Support (BLS) certification within 30 days of hire.
• Critical thinking skills, decisive judgment and the ability to work with minimal supervision.
• Ability to work in a stressful environment and take appropriate action.
• One-year previous experience preferred.
• Licensed Registered Nurse with the State of Tennessee or a licensed Registered Nurse from a Compact State with the proper Tennessee paperwork filed within 30 days of hire.
