Job summary

St. Croix Hospice in Minnesota is seeking a Registered Nurse Case Manager to provide 1:1, whole-person care in patients' homes, coordinating care with an integrated team.

The role includes assessments, care planning, administering medications, educating families, and on-call shifts; a valid RN license and reliable transportation required; benefits include health/dental/vision, disability, 401(k), and tuition support.

Qualifications

• RN with Associates, Bachelors, or Masters from an accredited nursing program.
• 1+ year of recent acute care experience; hospice/home health preferred.
• Ability to complete assessments and develop care plans.
• Strong patient and caregiver education and on-call readiness.

Responsibilities

• Complete initial and ongoing comprehensive assessments to determine hospice needs.
• Develop individualized care plans with patient and family, updating as needs evolve.
• Administer medications and treatments as prescribed.
• Educate patients and caregivers on care techniques, medications, diet, and symptom management.
• Rotate on-call duties and ensure seamless care transitions.
