St. Croix Hospice is seeking a compassionate Registered Nurse Case Manager to deliver 1:1, whole-person care in patients' homes across Wisconsin. You will coordinate care plans, educate families, and support transitions with empathy and professionalism.

Qualifications

• Associate's, Bachelor's, or Master's degree in nursing from an accredited program.
• 1+ years of acute care experience required; hospice/home health preferred.
• RN license required to practice nursing.

Responsibilities

• Provide 1:1, whole-person care to patients and families in their homes.
• Develop individualized care plans with patient and family; update as needs evolve.
• Administer medications and treatments as prescribed.
• Educate patients and caregivers on care techniques, medications, diet, and symptom management.
• Rotate on-call duties and support seamless transitions of care.
