Job summary

St. Croix Hospice provides compassionate end-of-life care and supports patients and families at home through a dedicated RN Case Manager. You will deliver holistic care, partnering with a care team to tailor plans that respect patient autonomy and dignity.

The role requires an RN license and at least one year of acute care experience; hospice/home health experience is preferred. Travel to patient homes is expected, with on-call responsibilities and a comprehensive benefits package.

Qualifications

• Current RN license required.
• 1+ year of acute care experience required; hospice/home health experience preferred.
• Hospice/home health experience preferred.
• Reliable transportation with valid driver’s license.
• Ability to develop patient- and family-centered care plans and educate caregivers.

Responsibilities

• Provide 1:1, whole-person care to patients and families in their homes.
• Coordinate individualized care plans with the patient and family.
• Support families through complex decisions with empathy and professionalism.
• Administer medications and treatments as prescribed.
• Educate patients and caregivers on care techniques, medications, and symptom management.
• Rotate on-call duties and ensure seamless care transitions.
• Collaborate with an integrated care team to optimize quality of life.
