Interim HealthCare in Portsmouth, NH is seeking an experienced Registered Nurse Case Manager to join our Home Health team. You will manage a dedicated patient caseload, provide skilled nursing visits, and coordinate care with physicians, therapists, and families in patients' homes.

Qualifications

• Active NH RN license required.

• Home health experience preferred; strong clinical competency considered.
• Previous nursing supervisory experience preferred.
• Wound care experience preferred.
• Current CPR certification.
• Valid driver''s license and reliable transportation.
• Strong clinical assessment, communication, organizational, and critical-thinking skills.
• Ability to work independently while collaborating with a multidisciplinary team.

What You'll Do

As an RN Case Manager, you'll take ownership of a dedicated patient caseload while providing high-quality skilled nursing care in the home.

Your responsibilities will include:

• Perform comprehensive nursing assessments and develop individualized plans of care.
• Provide skilled nursing visits and ongoing case management.

What We’re Looking For

• Current Registered Nurse (RN) license in the State of New Hampshire
• Minimum of two (2) years of skilled nursing experience preferred
• Home health experience preferred, but we are willing to consider strong candidates with relevant clinical experience
• Previous nursing supervisory experience preferred
• Recent wound care experience preferred
• Current CPR Certification
• Valid driver's license and reliable transportation
• Strong clinical assessment, communication, organizational, and critical-thinking skills
• Ability to work independently while collaborating effectively with a multidisciplinary team
