Job summary

Interim HealthCare Inc. is seeking a Registered Nurse (RN) for Home Health in Portsmouth, NH. This role focuses on one-to-one patient care in the home, with flexible scheduling and a diagnosable impact on patient outcomes.

The RN will assess conditions, create care plans, administer medications, and collaborate with the healthcare team to ensure high-quality care. Ideal candidates hold an NH RN license, have strong communication and assessment skills, and value compassionate, patient-centered

Qualifications

• Current Registered Nurse (RN) license in the State of New Hampshire.
• Minimum of one year of nursing experience within the last five years preferred.
• Home health experience preferred.
• Current CPR Certification required (monthly renewal classes available and paid for by Interim HealthCare).
• Strong communication and clinical assessment skills.
• Compassionate, dependable, and patient-focused approach to care.

Responsibilities

• Provide one-on-one skilled nursing care to patients in their homes.
• Assess patient conditions and develop individualized care plans.
• Administer medications, treatments, and other nursing interventions as prescribed.
• Monitor patient progress and communicate changes to physicians and healthcare team members.
• Educate patients and family members regarding treatment plans, medications, and self-care techniques.
• Maintain accurate and timely clinical documentation.
• Collaborate with interdisciplinary healthcare professionals to support patient goals and quality outcomes.
• Ensure compliance with all state, federal, and company regulations and standards.
