Responsibilities

• Perform comprehensive in-home assessments and skilled documentation (OASIS, SOC/recert/ROC, discharge).
• Deliver direct high-acuity patient care with emphasis on wound care.
• Build and manage plans of care with physicians; supervise LPNs and HHAs.
• Represent Adaptive to patients, families, and referral sources.
