Qualifications

• Current RN license in New Hampshire.
• Minimum of two years of skilled nursing experience preferred.
• Previous nursing supervisory experience preferred.
• Recent wound care experience preferred.
• Current CPR certification; valid driver's license required.

Responsibilities

• Coordinate and oversee care for a patient caseload in the home setting.
• Provide skilled nursing visits and case management services.
• Collaborate with physicians, therapists, and caregivers to plan and adjust care.
• Monitor patient progress and update plans of care accordingly.
• Educate patients and families to promote independence and better health outcomes.
• Complete accurate clinical documentation in a timely manner.
• Lead and collaborate with the interdisciplinary team to maintain high-quality care.
• Advocate for patients to ensure safety and continuity of care.

Interim HealthCare is seeking an experienced and compassionate Registered Nurse (RN) Case Manager to join our growing team in Keene, New Hampshire.

Position Details

• Location: Keene, NH
• Schedule: Monday-Friday | 8:30 AM - 5:00 PM
• Employment Type: Full-Time

What You’ll Do

As an RN Case Manager, you will coordinate and oversee the care of a dedicated patient caseload while providing skilled nursing services in the home.

Responsibilities include:

• Conduct comprehensive patient assessments and develop individualized plans of care.
• Provide skilled nursing visits and case management services.
• Coordinate care with physicians, therapists, caregivers, and other healthcare professionals.
• Monitor patient progress and adjust care plans as needed.
• Provide patient and family education to promote independence and improved health outcomes.
• Complete timely and accurate clinical documentation.
• Supervise and collaborate with interdisciplinary team members to ensure exceptional patient care.
• Advocate for patients while promoting quality, safety, and continuity of care.

Qualifications

• Current Registered Nurse (RN) license in the State of New Hampshire
• Minimum of two (2) years of skilled nursing experience preferred
• Previous nursing supervisory experience preferred
• Recent wound care experience preferred
• Current CPR CertificationReliable transportation and valid driver's license
• Strong assessment, communication, organizational, and clinical decision-making skills
