We are seeking a dedicated and compassionateRegistered Nurse (RN) Case Managerto join our home health care team in Colorado. TheRNCase Manager is responsible for overseeing and coordinating the nursing care of clients in their homes, ensuring high-quality, patient-centered services across a wide range of clinical conditions and populations.

This role involves comprehensive case management, including initial and ongoing assessments, development and implementation of care plans, coordination with interdisciplinary teams, and ongoing communication with patients and their families. The ideal candidate will demonstrate strong clinical judgment, excellent communication skills, and a deep commitment to patient safety and quality of care.

Key Responsibilities:

• Conduct comprehensive clinical and psychosocial assessments of patients in the home setting.
• Develop, implement, and continuously update individualized care plans.
• Coordinate care transitions and ensure appropriate discharge or service adjustments.
• Serve as a liaison between patients, families, providers, and community resources.
• Monitor patient outcomes and revise care plans based on evolving needs.
• Advocate for patients and promote adherence to evidence-based practices.
• Accurately document all case management activities and communications in the Electronic Health Record (EHR).
• Participate in interdisciplinary team meetings and contribute to quality improvement initiatives.
• Ensure compliance with all applicable federal, state, and local regulations, including Medicaid/Medicare requirements.
• Educate patients and their families on health conditions, medications, and available support services.

Qualifications:

• Active and unrestrictedRegistered Nurse (RN)license in the state ofColorado.
• Bachelor’s degree in Nursing (BSN) required; Master of Science in Nursing (MSN) preferred.
• Case Management Certification (e.g., CCM, ACM) preferred or willingness to obtain within one year of hire.
• Minimum of2 yearsof clinical nursing experience, preferably in home health, case management, or utilization review.
• Solid understanding of care coordination, home health practices, insurance guidelines, and community-based services.
• Excellent communication, critical thinking, time management, and organizational skills.
• Proficiency in EHR systems and Microsoft Office Suite.
