UP Health System – Portage is seeking a Clinic RN to provide compassionate, patient-centered care in a busy primary care setting. This full-time, 1.0 FTE role follows a Monday–Friday 8-hour day schedule and emphasizes teamwork to ensure high-quality outcomes for our community.

You will contribute by planning and administering care, documenting actions, and communicating patient status to the care team, physicians, residents, and families.

Formación

• Current Michigan RN license (or within 90 days of hire).
• Associate degree from an accredited nursing program; Bachelor’s degree preferred.

Responsabilidades

• Performs plan of care, including medication administration, specimen collection, clinical treatments and other medical care.
• Documents patient care provided.
• Monitors and communicates patient condition to care team, physician, nursing home, patient and family.
• Collaborates across disciplines to coordinate care, including transfers, discharge, referrals and psychosocial support needs.
• Assesses learning needs of patient/family and provides education appropriate to age, culture, condition and circumstances.
• Advocates for patient’s physical and emotional well-being.
• Delegates patient care activities to appropriately qualified staff.
• Performs prior authorizations on tests, procedures, medications, etc. as required by insurance.
• Performs other duties as assigned.

How You'll Contribute

A Clinic RN who excels in this role:

• Performs plan of care intervention, including medication administration, specimen collection, clinical treatments, as well as other medical care treatment.
• Documents patient care given.

What We're Looking For

Applicants should have a current Michigan RN license (or attain within 90 days of hire) and possess an associate degree from an accredited nursing school (bachelor's degree preferred). Additional requirements include:

• Basic Life Support certification is required (available upon hire)
• Annual Influenza Vaccination or Declination Required
