Elara Caring is seeking a Home Health Registered Nurse to provide one-on-one patient care in patients' homes across New Haven area and surrounding CT communities. The role includes a generous $5,000 sign-on bonus and full-time weekend shifts (Sat-Mon, 12-hour).

You will operate with clinical autonomy and support from an experienced team. The base pay ranges from $90,000 to $97,000 annually, with comprehensive benefits, professional growth opportunities, and travel across the assigned territory.

Qualifications

• Current, unrestricted RN license.
• Associate Degree in Nursing required; BSN preferred.
• At least one year of clinical nursing experience.
• Home health experience is required.
• Homecare Homebase experience preferred.

Responsibilities

• Assess patients and develop individualized plans of care.
• Provide skilled nursing services in accordance with physician orders.
• Monitor patient conditions, medications, and treatment progress.
• Educate patients and caregivers on treatments and disease management.
• Coordinate care with physicians and the Interdisciplinary team.
• Maintain accurate and timely clinical documentation.
• Help prevent avoidable hospitalizations and emergency room visits.
